Welcome to Anticoagulation Centers of Excellence
The Anticoagulation Centers of Excellence program was created in 2012 to help healthcare professionals provide the highest level of care and achieve the best possible outcomes for their patients on antithrombotic medications. Through access to evidence-based guidelines and constantly researched and updated best practices, tools and information, healthcare professionals will be able to improve outcomes through active participation in their patients' care. The Anticoagulation Centers of Excellence program offers a roadmap to achieving consistent, sustainable excellence in patient care.
There are two essential components to the Centers of Excellence program - an assessment tool to determine if your anticoagulation service is operating at the highest level, with recommendations and resources provided for specific areas that can be improved; and a comprehensive, searchable resource center that provides the guidelines and tools to improve towards excellence.
This Centers of Excellence program was created by the Anticoagulation Forum, the leading organization of healthcare professionals working to improve the quality of care for patients taking antithrombotic medications. More than 30 anticoagulation physicians, pharmacists, and nurse experts have worked together to create this resource.
There is no cost to participate in this program.

About AC Forum
Anticoagulation Centers of Excellence is a program of the Anticoagulation Forum. The Anticoagulation Forum is a multidisciplinary nonprofit organization of health care professionals that works to improve the quality of care for patients taking antithrombotic medications. The AC Forum works to minimize the risk and maximize the benefit of strategies used for the prevention and treatment of thromboembolic disease by providing education and networking opportunities for health care professionals, promoting the clinical application of evidence-based practices, facilitating research aimed at improving health outcomes, and informing healthcare policy, regulatory agencies, and industry about best practices. Founded in 1991 by Jack Ansell, MD, the organization has grown to over 6,000 members representing over 2,000 anticoagulation clinics throughout the world. Currently led by President Mark Crowther, MD, the AC Forum continues to be an important source of information and education for its expanding membership.
